A Clinical Trial of Cefortetan(YM 09330) in the field of Orthopedic Surgery





Abstract
The new cephamycin type antibi?tic Cefotetan (CTT, YM 09330) is very stable in respect to beta-lactamase, it possesse~ a supelative antibacterial activity on gram negative bacilli and anaerobic bacteria.
Total 18 cases of musculoskeletal Sepsis were treated with Cefotetan at the Department , of Orthopedic Surgery, Kyungpook National University Hospital, Taegu, Korea from March, 1985 to May, 1985.
The clinical results were as follow:
1. Cefotetan was administered by intravenous volus injection at daily dose 2 or 3 grams (lgm x 2 or 3 times) for 4 to 25 days. In children, it was given as 50-100 mg/kg daily.
2. Bacteriological results showed that Cefotetan proved effective against gram positive cocci (Staph. aureus, Staph. albus) and gram negative bacilli (Pseudomonas, Klebsiella, Bacillus subtilis). Average effective rate was 73.2% (sensitive in 13 cases from 18 isolated organisms). I
3. Cefotetan was administered to 15 cases with surgical infection (acute osteomyelitis, chronic osteomyelitis, pyogenic arthritis, cellulitis, posttraumatic infection) and 3 eases with preventive measure on open fractures. Of 18 patients receiving Cefotetan alone, 83.3% responded with a significant clinical improvement.
4. Side effects were not observed except slightly elevated values of SGOT, SGPT and alkaline phosphatase in 2 cases, but within normal range. No other abnormal laboratory findings were shown on 3rd, 7th land 14th days after drug administration.
